What Your DMCA Notice Must Include
To be valid under 17 U.S.C. § 512(c)(3), your notice must contain all of the following:
- 1 Physical or electronic signature of the copyright owner or an authorized agent acting on their behalf.
- 2 Identification of the copyrighted work you claim has been infringed, or a representative list if multiple works are covered.
- 3 Identification of the infringing material — specifically the Facebook URL(s) involved and the nature of the infringement.
- 4 Your contact information including your full name, mailing address, phone number, and email address.
- 5 Good faith statement that you believe the use of the material is not authorized by the copyright owner, its agent, or the law.
- 6 Accuracy statement — a declaration under penalty of perjury that the information in your notice is accurate, and that you are the copyright owner or authorized to act on their behalf.

If you believe your content was removed in error, you may submit a counter-notice. A valid counter-notice must include: your contact information, identification of the removed material, a statement under penalty of perjury that the removal was a mistake or misidentification, and your consent to the jurisdiction of the relevant court.
